What is an eForm 7 and what do I use it for?

The eForm 7 is an online method of reporting injuries/illnesses to the WSIB in an efficient and timely manner.

Why do you have a new eForm 7 online reporting tool?

We have improved the format making it easier to submit and track your claim online.

How much does it cost to use eForm 7?

There is no charge for using any of the WSIB’s online eServices. You just have to sign up for our e-Services and sign on to use the e-Form 7.

How do I know when a claim has been received?

You will get email notification that the claim has been registered after you submit the claim through the eForm 7. Once the claim status is available you will be notified again to check the status on your secure eServices homepage.

How do I know when a claim has been accepted or rejected?

Shortly after you receive confirmation of your claim submission you will be updated by email to check the claim status. You can then log into our eServices and check your secure eServices homepage for the status of that claim.

When can I expect to receive a claim number?

You should receive a claim number within one (1) business day.

Do I have to keep a record of the report for my files or does the WSIB keep it on file?

A record of your eForm 7 claim and status is kept for 90 days on your secure eServices homepage. You can also save or print a PDF copy from the file for your own records.  

What do I do about additional information and attachments when I use an online eForm 7?

Documents can be attached directly to the claim file during the registration process. All documents attached in this way will become part of the claim electronically.

Do I have to use the online eForm 7 to report an injury or illness at the worksite?

No. You can still mail, fax or hand deliver your Form 7. The new eForm 7 offers employers a quick, secure, reliable and convenient way to submit a claim.

Does this change my reporting obligations?

No. The eForm 7 makes it easier to meet your reporting obligations. Employers still must complete and submit the Employer’s Report of Injury/Disease (Form 7) immediately after a work incident.

What happens if I don’t make a report on time?

The WSIB policy sets out administrative penalties of $250 for each instance of late reporting. And a penalty of $1,000 for accidents reported after more than 30 days. The penalties for individuals who do not report and are convicted of an offence, are a fine of up to $25,000 or a prison sentence of up to six months or both, and for corporations a fine of up to $500,000.

How quickly should this report be sent to the WSIB?

You are required to file a report within 3 business days. The new eForm 7 offers employers a quick, secure, reliable and convenient way to submit a claim.

Do I have to provide a copy of the report to the worker?

Yes. The eForm 7 can be saved as a PDF and printed out to share with the worker.

Can I still use my B2B service to submit Form 7 reports?

Yes. If you are currently using a B2B service you can continue to use that service to file your Form 7 reports.
