How do I request government documents?

Workers

Workers can obtain a copy of their claim file at any time without going through the formal Freedom of Information and Protection of Privacy Act (FIPPA) request process. To request your file, fill out the Worker Request for Copy of Claim File (146k, pdf) and return it to us. We will send you a copy of your claim file within 30 days.

How the General Public can make a FIPPA Access Request

You can make a request under the Freedom of Information and Protection of Privacy Act (FIPPA) by filling out a FIPPA Request Form. You can also make an access request in the form of a letter, identifying the information requested and indicating that the request is made under FIPPA.

  • A $5.00 application fee per request is required for FIPPA requests
  • The application fee is payable to the Workplace Safety and Insurance Board
