Document upload tool

The WSIB is making it faster and easier for you to send us claims information.

Complete and submit forms, letters and documents related to WSIB claims online.

Here’s how it works:

Completing forms online

  1. Go to the relevant forms page to find the form you need:
  2. Complete the form by typing required information into the fillable fields.
  3. Save the form using the “Save” button at the top of the form.

To send a copy of a printed document or letter, you can scan or take a photo of the document.

Uploading forms and documents

  1. Go to the document upload tool
  2. Enter the requested claim information
  3. Select the documents that you want to upload. You can either drag and drop the files from your desktop, or select “Browse” to find the files. Once you have attached the files, click “Submit”.
  4. You can upload up to three documents at a time. If you have more documents to upload, you must submit them separately. If you need to upload documents or forms for more than one claim, you must do separate submissions for each individual claim.

  5. Check the file names to make sure you have attached the right documents. If you have attached the right documents, agree to the declaration and click submit.
  6. Once you click “Submit” you will see a confirmation screen. You will also receive a confirmation email if you opted to receive one on the previous screen.
