What's an eForm 7?
Our new and improved eForm 7 makes it even quicker and easier to report a workplace injury /illness online. Submit online and report on time! Now you can:
- Skip over pre-populated and default fields
- Attach important supporting files in PDF format
- Save and bookmark an eForm 7 in progress
- Receive a confirmation number of your submission immediately and an electronic notification of claim number and status
- Review the claim status for all eForm 7s submitted in the past 90 days online
- Receive claim number within one business day
- New! Upload and electronically submit additional documents to your claim for up to 90 days
New Feature for eForm 7
Now you can upload and electronically submit additional documents to the WSIB and have them go straight to the claim file.
Once your claim number is on file, you’ll have 90 days to upload and attach additional documents relating to the claim. To upload, you’ll just need to click on the "Add" button next to the claim number, and you’ll be able to submit a scanned document that will go directly to the claim file.

How can I submit a Form 7 online?
There are two simple steps to access eForm 7:

Important information about sign-up or enrolment for eServices
After you have signed up or enrolled for a new eService, please wait for our confirmation email. You can then access the eService using the website link we provide in the email.

Schedule 2 Employers (firm number begins with "8")
Fill in Rate Group Number 999 and Classification Unit Code 0000-999 to correctly complete the eForm 7.
Employers using the Business to Business Form 7 must sign up if they also wish to use eForm 7.
