eForm 6 tips

  • Enter your claim number and date of birth as mandatory fields
  • Use only 8 digits to correctly enter your claim number--do not enter anything else in the claim number field
  • Use this format to correctly enter your date of birth: dd/mm/yy e.g. 28/04/81
  • If you do not have a claim number, contact General Inquiry at 416-344-1000 or 1-800-387-0750
