This section contains many of the forms provided by the WSIB. These forms can be filled out online and have simple prompts and help, but remember that you must print, sign, and then send them to the WSIB.

Technical Problems
Select WSIB forms have encountered intermittent problems opening in the Adobe Acrobat 9.0 Reader format. The WSIB has isolated this problem and it is being corrected. If you encounter a problem, please contact the WSIB at wsibcomm@wsib.on.ca and we will get back to you and help you obtain a hard copy version as a temporary measure.

Completing Forms
To complete a form:

Click your cursor to the right of the START HERE pointer “>”, and start typing.
TAB to the next blank space. If you miss or need to change an item "SHIFT / TAB" back to that space, and make your change.
To clear the form and start again, click the “Reset” button at the top of the page.
To enter an "X" in a check box, just click on the box.
Enter dates as in the example: 23 Aug 2007.
If the cursor disappears, just click it back to the last space you entered.
After making your last entry, hit TAB again to ensure your entry records on the form.

After completing the form, please ensure that you clear your browser’s cache.

To clear your cache:
1. On the Tools menu in Internet Explorer 6, click on Internet Options.
2. On the General tab, in the Temporary Internet Files section, click on the Delete Files button.
3. Click on Ok and then click on OK again, at the bottom of the box.

When you have completed the form, be sure to print, sign, and fax or mail it. The web page does not electronically submit forms.
